KIPP Minnesota, along with Community Care Clinics and V3 Sports, is hosting COVID-19 vaccine clinics for KIPP students, staff, and their families (ages 5 and up), and any other community members. Moderna and Pfizer vaccines will be available. No appointments necessary. Please bring your vaccine card unless you are receiving your first dose.

Pfizer Vaccine (1st or 2nd dose): For individuals 5 years and up
Pfizer Booster (5 months after 2nd dose): ONLY for individuals 12 years and up
Moderna Vaccine (1st or 2nd dose): ONLY for individuals 18 years and up
Moderna Booster (6 months after 2nd dose): ONLY for individuals 18 years and up

NOTE: Anyone who received one dose of the Johnson & Johnson vaccine can get a Pfizer or Moderna booster two months after their Johnson & Johnson vaccination.

CONSENT FORM: No individual under the age of 18 will be vaccinated without a signed consent form by a parent or guardian, even if this is not their first dose.
